Information this website processes
The public website is designed to provide clinic information and direct visitors to online scheduling. Function Health does not ask visitors to submit medical information through this website.
Scheduling and paperwork
Scheduling is provided through Acuity/Squarespace, and patient paperwork is provided through a separate clinic form system. Information entered into those services is governed by their respective privacy practices and the clinic’s patient privacy procedures.
Measurement
If analytics or advertising measurement is enabled, it will be limited to general site activity and completed-booking events. The site will not intentionally send names, phone numbers, appointment notes, symptoms, or patient-entered health information to advertising or analytics platforms.
Contact protection
The text number and email address are delivered after visitor interaction to reduce automated collection. Basic request information may be processed temporarily to limit abusive automated requests.
